Patient FAQs

This page provides answers to common questions about our electronic patient-reported outcomes, which are collected through surveys sent via email and text message. If you still have questions about this process, please email Jennifer Minock at cjennife@umich.edu.

Is this email or text a scam?

HMS has partnered with Qualtrics to safely and securely collect patient information. Qualtrics sends messages to patients via text and via email. HMS is working to improve the quality of care and safety of people who had a hospital stay. If you receive a message like the ones pictured below – please respond to assist with these efforts.

What kinds of questions will I be asked?

If you’re invited to complete an HMS survey, you might be curious about the types of questions and how much time it will take. The survey gathers insights on topics such as side effects from prescribed antibiotics, any challenges you’ve faced since discharge, your physical recovery, and whether you’ve returned to work. Most patients find the survey takes just 5–10 minutes to complete.

Why should I respond to the questionnaire?

Your recovery is a personal journey, and your voice is essential. By sharing your experience—including adverse reactions, quality of life, and both the challenges and successes—you help us better understand and improve the care process.

When will I be contacted for questionnaires?

30 days from discharge for patients that had pneumonia or a urinary tract infection, and 60 days from discharge for patients that had sepsis.

Will my information be protected?

Yes! It is placed in a secure registry.
